  • ROSS provides an objective outside view to examine the accident, machinery operation, procedures, guarding, and safety controls.
  • A root cause analysis will be provided looking at the various influences that resulted in the incident.
  • ROSS can provide a variety of suggestions for improvement and next steps.
  • Some short term steps can usually be taken to get machinery back into operation without exposing personnel to additional hazards.  This could include re-installing guards and procedural changes.
  • Medium term steps may be able to be implemented to begin reducing risk more systematically.  Modifications to safety systems to improve its reliability due to issues found in integration or lack of safety rated outputs or additional guarding to remove existing gaps.
  • Long term steps may be required such as a full risk assessment to truly understand the hazards and remediation required to get the machine into full compliance.

When an accident occurs on a machine there can be many potential causes interacting.  Having an objective outside viewpoint from a machine safety expert can be beneficial to find the root cause without any internal company biases that may exist. By examining how procedures, personnel actions, guarding, and the safety system interact there are frequently shortcomings found in a few areas if not all.  A holistic look at the accident can provide insight on not just improving the safety of that machine but provide insight on potential issues that may exists systemically within the facility or corporation.
